This agreement outlines the purpose of mentoring, confidentiality, professional responsibilities, limits of advice, and expectations for both parties.
Mentoring supports clinical reasoning and professional learning. It does not replace patient-specific assessment, clinical supervision, workplace policies, hands-on competency development, or professional judgement.
Please email your signed agreement to [email protected].
Once we receive your signed agreement, we’ll send you the booking link for your mentoring session.
Thank you — we look forward to working with you.
